Peripheral Nerve Stimulation (PNS) is an innovative, minimally invasive procedure aimed at managing chronic pain caused by neuralgias and neuropathy. Neuralgias refer to severe pain along the path of a nerve, while neuropathy encompasses conditions that result from damage to, or dysfunction of, the peripheral nerves. PNS is designed to alleviate this pain by disrupting the pain signals being sent to the brain.

The procedure involves the implantation of a small electrical device, known as a neurostimulator, near the peripheral nerves. The neurostimulator generates mild electrical pulses, which are delivered to the nerves via one or more thin, insulated medical wires called leads. These electrical pulses interfere with the nerve’s ability to send pain signals to the brain, essentially ‘scrambling’ the signals, which can result in a significant reduction in pain for the patient.

The PNS procedure typically begins with a trial stimulation period, during which temporary leads are placed under the skin and connected to an external stimulator. This allows the patient and physician to assess the effectiveness of the therapy and adjust stimulation settings to optimize pain relief.

If the trial stimulation proves successful, the patient may proceed to permanent implantation. This involves placing the neurostimulator under the skin, typically in the lower back or buttock, and connecting it to the leads placed near the targeted peripheral nerves. The entire procedure is usually performed under local anesthesia and sedation.

Post-procedure, patients can control the intensity of the stimulation via a handheld programmer, allowing them to adjust the level of pain relief to their needs. The neurostimulator can be deactivated or removed if necessary.

As with any surgical procedure, PNS carries potential risks and complications, including infection, bleeding, and possible lead migration. However, for many patients suffering from chronic neuralgias and neuropathy, PNS can provide significant pain relief and improved quality of life.
